The “Gift of Health” gift certificate can be used in the Department of your choice at the Maximized Health Wellness Center (Allergy & Asthma Department or Chiropractic & Physiotherapy Department).  Only one gift certificate can be used per person.  The gift certificate can be forwarded to another person that you feel would use it.  More than one gift certificate can be used for additional family members, i.e. family checkup.  The gift certificate can not be redeemed for cash and has an expiration date of January 31st 2010.
